Audio stops working if you change outputs during playback

Description
On my Macbook pro running Snow leopard and my Macbook running leopard I have found an easy to reproduce bug.
If you are listening to audio with headphones and then unplug them the video will continue playing with no audio. If you plug the headphones back in the audio will still not resume unless you pause and unpause the video.
I've noticed this issue since rev12 however I'm not sure if it existed before then.

Just as an additional, perhaps, helpful point...
I was running rev12 on Leopard with no issue switching between headphone/speaker outputs and sound not working, but have noticed this issue now I'm on Snow Leopard running rev13 64bit with ffmpegMT.

Fixed in rev14-test1.
The issue was caused by MPlayer having no run loop when running in the back of MPE (without a window). Since I had to touch the coreaudio driver, I've added audio output device selection as a bonus.

Issue still exists in rev14-test2.
